New Patient
FORM Physical Therapy provides physical therapy services in Roswell, Georgia. We’re dedicated to providing a caring environment for patients to treat pain and promote healing and strength. If you’re a new patient, please review the below information prior to your first visit with us.
Your First Visit
If possible, please fill out the online forms below. If you are unable to fill out and print the documents online, please arrive 15 minutes early for your evaluation.
Once you arrive, we will have your file set up, and you will complete any additional required paperwork.
Please bring with you a government ID, a copy of your insurance card, and a list of any prescriptions or over-the-counter medications that you take regularly.
Your physical therapist will spend time with you to conduct a thorough examination and evaluation of your injury or condition and recommend a path forward for treatment and recovery.
You can also expect your first therapy session during this time, so please wear loose-fitting, comfortable clothing.
At the end of your first treatment session, the physical therapist will discuss the frequency of your visits and the expected duration of your treatment plan.
